Longarm Quilting Request Form

The first step in the process of hiring us to longarm your quilt is to choose a pantograph quilting design and fill out this request form! I will contact you with my mailing address (be sure to check your spam/junk folders for my email). I will email you when your quilt arrives to my home. When I complete your quilt, an invoice will be generated and due in 7 days (unless we make other arrangements).

If you would like to drop off your quilt in person, make an appointment online! No need to fill out the request form, I’ll get all the information during your appointment.

Quilt Dimensions (in inches) *

Quilting fee of $0.025 is based on the square inches (LxW) of your quilt top. I have a minimum quilt fee of $40.

Quilt back will measure 8" longer and wider than your top. Please make sure your quilt back is pressed and SQUARE. A great quilting job starts with a square quilt back.

Thread Color *

I use Glide Thread and have many cones in a variety of colors and shades. I can match your quilt background or a color in the fabrics you use.

I offer batting for sale. If you send batting, please send a good quality batting for the best results!

Extra Wide Minky Quilt Backs Available (https://sirenstitching.com/?page_id=2916) - Let me know if you want to use my fabric. It will be added to your invoice.

What is your return address for USPS mail? *

I will use this to estimate postage and prepare your shipping label when the quilt is complete.

My policies: I will not accept quilts with backs that are not AT LEAST 4" larger ALL AROUND the quilt top. If you use scrap around the sides, it could end up in your quilt. Queen and King size quilt tops should use 108" backing fabric or have a horizontal seam. I can accommodate quilt backs up to 120" wide. Please read my FAQs if you are sending minky. I have a $40 minimum quilting fee. I do not offer binding or quilt repair. *I agree.
